Head of Aging Care 1 or 2

Starting salary: $31,908.50 or $34,301.64

This care manager will serve as the main information and reference person for the agency. This requires answering calls and assisting walk-in clients who request information about agency services. This care manager will also perform a reduced workload. This would involve interviewing and assessing consumers in their homes and developing an appropriate plan of care.

REQUIREMENTS (Aging Care Manager 1):

  • Must meet PA residency requirement.
  • Three years as an aging case assistant 2; WHERE
  • A bachelor’s degree that includes or is supplemented by twelve (12) credits in social sciences, behavioral sciences, human services; WHERE
  • An equivalent combination of experience and education.
  • Must be able to perform essential job functions.
  • Individuals hired as Aging Care Manager 1 are promoted to Aging Care Manager 2 when eligible.

REQUIREMENTS (Aging Care Manager 2):

  • Must meet PA residency requirement.
  • Six months as Aging Care Manager 1 or County Caseworker 1; WHERE
  • Successful completion of the County Social Casework Intern program; WHERE
  • Six months of professional experience in social services or nursing; and a bachelor’s degree in social sciences, behavioral sciences, social services or a closely related field; WHERE
  • An equivalent combination of experience and education.

Aging Care Management Supervisor 1

Starting salary: $40,206.43 – Depends on experience

Oversee all care management and casework activities with Care Managers in programs including OPTIONS, Person Centered Counseling (PCC), Caregiver Support Program (CSP) and Caregiver Determination. Functional Eligibility (FED).

Aging Care Manager 2, Protective Services

Starting salary: $36,874.26 + $275/week on periodic call

Assesses the medical, social, and psychological needs of older adults, investigates allegations of elder abuse, and provides services to detect, reduce, correct, or eliminate risks associated with all types of elder abuse identified through investigations. The job is to act to ensure the well-being of older people, as well as their living situation, their financial situation, their personal safety and their quality of life. Also takes “on call” referrals after normal working hours, rotating weeks with other staff.

OPTIONS: Assesses needs and manages social services for seniors and their families to improve their quality of life. The work involves assessing the social contacts and supports, activities of daily living, cognitive functioning, physical health, and economic and other resources of older adults in order to develop plans for their care. Includes coordinating plans with other agencies to refer clients to additional services and to help seniors use available services to meet their needs.
